For some reason in Revit, the cosmo browser is empty. Opens fine in 3ds max, and CCB service is running.

Please, uninstall the Cosmos Browser manually from Windows Add or remove programs panel (as described by Kalina in this forum thread)
Then reinstall V-Ray for Revit and it should be fine.
