Here's the final image from a fairly recent project: (it looks a bit crap as it's been heavily down-sized to fit the post - a higher res can bee seen here. Original width was around 20K!)
This image was done for the Children's Hospital here in Vancouver, BC for a campaign to raise money for expansion (the headline was: "We have 1/2 the space we need"), and it was intended to be largely a retouching job - but evolved into being quite a large CG project for me.
I ended up creating the environment and most of the room around the people. The medical equipment is mostly CG too (bought from Digital-X models and I rebuilt the materials). The final res was pretty crazy - about 20K per side (this is a very narrow crop of the whole image) - so the textures had to be quite high-res. I used Arroway for most of it. Also used Railclone Pro for the conduits and ducting stuff, which was a huge time-saver.
