Overwrite a huge EXR because I forgot to change/disable the output for a test render, or launched two renders with the same path.
        Same for IR/LC maps.
        Forgot to activate displacement.
Launched animations from perspective view/wrong camera.
Left an environment background image/Sky, we always add a bg in post.

well I cant remember details about such things.
Once I had to do a car render which was 8000x4000 (approximatly) pixels for print. Everything took too much time and we desided to render it in strips. I sent 11 strips to the 11 machines we had. And went home. When I came the next day, it was all rendered and merged and it looked perfect. That same morning we had to deliver and we did and it was a success. I cant stress how much everything was hanging by the thread. And how much strip rendering has issues on all fronts. Or used to have.

But on the side note. I had few instances where I have rendered a shot, and moved on to the next one reusing the scene from pervious one. Upon completion of a shot setup I sent it to the farm, forgetting to change the path to the new directory of output images, and overwriting previously rendered shot with different shot frames.

2 different teams with different software, rendering equipment shared. (can lead to inter-personal problems as well!!) Just shut my stuff down to get their stuff going on.
            Missing the Alpha
User not allowed to save to path.. ( we had a few server hardware/software changes in the last couple months, some permissions got lost in the process..)
