When I have a situation like that, I put together a fee for making the changes so they can decide if they want to proceed with the revisions. Usually it's something like 1/3 of the original fee, so most times I get the go-ahead. I tell them the scope has changed, and here's what this additional work will cost. Seems having a fee up-front helps the client figure out how to proceed, and still covers my cost, mostly.
