Are you boosting your reflections really heavily in photoshop? Noise is normally caused by either very grainy reflection settings in the material, Or very low samples on the light that causes the spec / reflection, or if the material isn't being hit directly by a light, it's reflecting an object that's very grainy.

Turns out is was some VR IES lights nearby, causing the issue. I turned off the "affect specular" setting to them and that appears to have fixed the issue. Thanks. Would have never thought of that!
