Subdivision modelling with some noise? Easy.

I guess it depends on how close you'll need to go to the object, as the amount of detail and work involved may vary considerably. if it has to be really detailed then I guess the best approach would be to just sculpt it.

It looks like a block of fudge.

I'd probably do a combination of quick modeling & noise modifiers - and then additional materials for bump/disp depending on distance, possibly from that same photo to push the effect further.
