Since moving to VRay 3, whenever I load a scene using proxies, more often than not the proxies are shown as (solid) bounding boxes, even though the proxy itself says it's in "preview from file" mode. Switching to a different mode and back again fixes the problem, but this is a bit tedious to do each time I load a scene! Is this a known issue?

Had the same problem, didn't find a solution apart from writing a script that re-sets the display mode.

It seems like this might be a Forest Pro problem. I saved out one proxy and when I re-opened it, it was ok. I went back and deleted one Forest Pro object and it was still busted, so I deleted all of them, re-opened and it was fine.
We're using Max 2015 64 Design with VRay (whatever the latest is - 3.3?). All the proxies were saved out with previous versions.

ot but there is a simillar problems with lines viewport display.
If you have renderable lines and you set them to not to show in viewport (uncheck "enable in viewport") and then render, after render finishes all lines thicknesses will be displayed in the viewport as if "enable in viewport" is checked.
Very anoying if you have lots of renderable lines.
