sometimes this is cause by very bright elements in the scene, i would suggest, that you successive start to hide suspect objects (like lights, emitter materials ...)until you find the culprit.
You can also try if the activation of the "sub pixel" option helps...
Like Andy said, sub pixel generally does the job for me as well. Also, off topic, You can also enable clamp output if you are not saving to a file format like hdr or exr (since i see you have it disabled)
