At the download site there are two Batch render downloads - one that says “Batch render for V-Ray for Rhino 4” and one that says “Batch render update for V-Ray for Rhino 4 SR1” - however the first download is posted at a later date than the second, so my question is, do you have to install both or is the SR1 patch integrated in the first download?

They are two VERY separate plugins and the SR1 version does not depend on the first version. The dates for the posting are merely when they were added (or modified) to the the download page.
