Mysterious vray plane

Just checking I'm not going insane.. a vray plane keeps coming back at random times to haunt me. Is it Phoenix creating this and what triggers it. I delete it and it comes back while I'm working and never notice whats creating it

Unless it is a max2016/vray feature?

its a new vray plane that seems to create it self while I'm using Phoenix. I assume its phoenix as not had it in scenes where phoenix is not used.

i wonder what it can be. phoenix creates automatically foam shaders when toy turn on the foam, but can't imagine error that produces planes. is it happening only in this scene?
