U want to try setting this more like 4/25/005, see if that makes any difference. To be honest, I can't make out anything you have in there though. Let me see if I can explain this correctly and if myself have this right:
You don't have enough samples going in that direction. So you can increase sampling to a million, but you still don't have enough being added to that area, because VRay does not consider you need any there. Hence, my option for putting Min of 4 instead of 1. That means everywhere you will get an increased sampling, hence my option for bringing the Max down to 25. You may need more, but I think that may be enough and my reason for bringing Noise to 005 is because you are only bringing it down to 003 because you are trying to clean it up, but you can't since you don't have enough samples being sent in that direction. Anyone else to add if I'm in err somewhere here.

ALso try limiting secondary ray intensity, enabling sub-pixel-mapping and clamping, and try another AA filter like cubic or quadraticSoftware:

try to DISABLE subpixelmapping because it seems to have issues in builds lower than 3.3.03...
what does the renderelement samplerate mean? is it blue or red?

That is one crazy samplerate pass...
Do you look through some strange glass maybe? Do you have atmospheric effects enabled with some strange settings? Some crazy post processing or sharpening filters active?Software:

That must be one hell of a scene. I personally would like to take a look at it. Not sure what's making it so. Cause if it errs due to memory, then there is something wrong with your scene or materials. And like he said below, that's one hell of a sampleratepass for those settings. Curious, does IR work any better on this scene?
