We are able to reproduce it with the specific 3dm file you sent us (I'm not saying otherwise), however, we cannot get the same behavior if we try to make a proxy template file from scratch.
We can, of course, schedule a remote session and investigate a bit further the creation process of those files and troubleshoot on the spot - if that is alright with you, you can get in touch via our contact form to do so.Nikoleta Garkova | chaos.com

Ok, we can do this, but for me it is not so much about the creation process anymore, since I'm using the newest update of Vray 5 now and I suppose that the bug will not appear here. But remote session sounds ok...I'll use the contact form.

I'm having the same problem. proxies are rendering correctly but in Rhino viewport some of them are wrongly positioned, scaled and also distorted.
When the fix will be available? This is quite urgent.
We'll do our best to put out a hotfix that resolves it as soon as possible.

This issue is fixed in V-Ray 5 for Rhino, update 1.3

I'm sorry but with this update i can't see nothing in Rhino viewport (everything is render correctly but all objects are like hidden in rhino, not only proxies...)
