CUDA version was the one I used - it was the test scene supplied at the beginning of this forum thread.
Vray version 3.00.03
GPU is a Quadro 4000 with latest certified drivers, version 331.65
Only tried to render with this machine/version as a test for my graphics card
I just got the same result as Tricky, using a Nvidia Quadro K4000 card.
Using V-Ray Standalone with the test scene, for both -rtEngine=5 and -rtEngine=3 I got the following error in the CMD window:

Do you get this error only with this test scene? Does it work with other scenes?
Could you please post the full log from the Standalone console window?
I get the same result with the test scene or with my own scene. I created a very simple scene with only one sphere and tried this in both the standalone and 3ds Max GUI version. Both resulted in an unhandled exception. Below is the log from the Standalone console window using the test scene. With the test scene, if I set -rtEngine = 0 or 1, everything works. Values 5 and 3 fail with an unhandled exception. A Vray.dmp file was also created. If it helps, I could send you that.

It seems to be because V-Ray can't initialize the CUDA system (where it says initDevices() failed). I'm not sure why this happens yet, we will need to find some ways to debug this here.
It looks similar to another case where the problem comes from the Environment variables.
Please try to do a Right click on OpenCL Device select tool and choose Run as administrator. Then make sure that your graphic card is selected and no errors are shown there.
Also it will be helpful if you can post or send to: support@chaosgroup.com the output from the command "set" (without the quotes) in CDM. We need to see only the V-Ray related environment variables, so you could just create a txt file or a print screen to catch them.
I realized what was causing this problem. I was accessing the computer with a Remote Desktop Connection. I tried the test scene in a command prompt while sitting directly at the computer, and it seems to work fine. So either the OpenCL device does not initialize properly while using a Remote Desktop Connection, or perhaps the environment variables are set differently when logging in remotely.

Nvidia GPU cards don't load properly their drivers when there is no monitor connected or via Remote Desktop. Try connecting with 3rd party remote software.
If it was that easy, it would have already been done
